Overview

The Sub-Acute and Long-Term Care sector is rapidly growing, offering excellent opportunities and a flexible work-life balance, with no on-call duties or weekend shifts!

We are looking for a healthcare provider to fill a part-time role of 32 hours per week, divided between two facilities in Frederick, MD, and Walkersville, MD: Autumn Lake Healthcare at Ballenger Creek and Autumn Lake Healthcare at Glade Valley. This position is open to either a nurse practitioner or a physician assistant, with a minimum of one year of relevant experience required.

The hourly wage for this role ranges from $56.73 to $59.13, along with an incentive compensation bonus program that has no cap.

The Advanced Practice Provider will report to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaboration with the Center Leadership Team to meet specific goals and priorities. Additionally, the Advanced Practice Provider will work alongside the Medical Director and/or Attending Physician to deliver patient-centered medical care.

Key responsibilities include:

• Providing primary and urgent care services to residents of the nursing center.
• Collaborating with other medical providers to ensure high-quality healthcare delivery.
• Working with Center Leadership to fulfill our mission of enhancing lives through compassionate healthcare.

To qualify for this position, candidates must meet the following criteria:

• Graduation from an accredited School of Nursing, with a valid registered nurse license.
• Certification as a Nurse Practitioner, having completed a Master’s Degree in Nursing with a focus on Older Adult or Family Nurse Practitioner (FNP) specialties. Other specialties may be considered based on experience and organizational needs.
• For Physician Assistants, graduation from an ARC-PA accredited program and successful completion of the PA National Certifying Exam (PANCE) is required, along with state licensure and NCCPA certification.
• A minimum of two years of experience working with adult or geriatric populations is preferred.
• In states where permitted, an unrestricted professional license and DEA registration are required for both APRNs and PAs.

Salary Range: USD $56.73 - USD $59.13 per hour.
