About the Role We’re looking for a dedicated Registered Nurse-Physician Practice to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you’ll organize and administer individualized, professional patient care to patients of a primary care practice. You will provide nursing care as directed by the provider and in accordance with the objectives, policies and procedures of the hospital and the state Nurse Practice Act. You will provide clinical leadership to other members of the care team and create a caring and healing environment where patients and their families are the center of the care process. What You’ll Do Patient Assessment: During patient visits, assess the patient’s physical/psycho-social status, review medications and confirm medication reconciliation as appropriate. Obtain vital signs, height, and weight. Provide results of assessment to the provider and document findings in the patient’s medical record. Analyze relevant data and integrate assessment results into care planning, goals and outcomes. Patient Care: Collaborate with the provider to formulate and implement a plan of care as appropriate based on assessment and diagnosis. Refer patients to case manager/social worker to ensure access to additional services and resources. Ensure consistency in standards of care and that the provision of care meets all applicable Joint Commission standards to ensure patient safety. Triage calls from patients, family members and community health providers. Engage in relationship-based care to create a caring and healing environment. Advocate for patients and families. Communication: Apply patient-specific communication skills in culturally sensitive interactions and population/age-specific needs. Accommodate language barriers and sensory and cognitive deficits as needed. Communicate information to the provider that is pertinent, timely and concise. Manage conflict among team members in a direct and effective manner. Patient Education: Provide information to patient/family with consideration to individual needs. Assess for barriers to learning and motivation. Consult with other care team members to enhance patient and family education. Participate in developing patient education materials. Review patient educational materials with patients and their families. Provide patients with appropriate teaching materials and resource booklets. Clinical Leadership: Delegate and/or assign care consistent with role, scope of practice and abilities of other clinical care staff. Actively participate in problem solving. Provide feedback to other care team members to enhance education and knowledge. Coordinate care across the team. Conduct research as needed to respond to patient questions and concerns. Use critical thinking to analyze data to support clinical decision making. Demonstrate the ability to solve problems, reason and learn using evidence-based practice. Ensure staff practice within scope of practice. Documentation: Ensure all documentation is accurate and in compliance with institutional standards.
