RN Case Manager – Pediatric Home Health (Field-Based) $70,000–$80,000 Salary | Company Vehicle Provided | Full Benefits Clarks Summit, PA | Full-Time CareGivers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one-on-one pediatric home health visits across: Lackawanna, SusquehannaCounties This is a field-based, salaried role with a company vehicle provided. Schedule & Travel Expectations Full-Time | 40 hours per week 5 days per week | Monday–Sunday | 40 hrs a week (Flexible schedule) Daily travel across assigned coverage counties based on patient needs This role requires flexibility and comfort with travel within the listed counties.
