Village Green is looking for a compassionate, organized, and resident-focused Social Services Director to join our team. This position plays a vital role in advocating for our residents, supporting families, and helping each resident feel heard, valued, and at home.

What You'll Do:

• Serve as an advocate for residents and help ensure their psychosocial needs are met

• Complete social services assessments and develop individualized care plans

• Provide emotional support and assistance to residents and their families

• Coordinate and participate in care conferences

• Help residents adjust to their new environment and changes in their care needs

• Address resident and family concerns and work toward positive resolutions

• Maintain accurate and timely documentation in accordance with regulatory requirements

• Assist residents and families with discharge planning and community resources

• Collaborate with nursing, activities, therapy, dietary, and other departments as part of the interdisciplinary team

• Support resident rights, dignity, choice, and quality of life

• Participate in facility programs, meetings, and quality improvement initiatives

What We're Looking For:

• Experience in social services, healthcare, long-term care, or a related field preferred

• Strong communication and interpersonal skills

• Compassionate and empathetic approach

• Excellent organizational and problem-solving abilities

• Ability to work effectively with residents, families, and an interdisciplinary team

• Knowledge of long-term care regulations and resident rights is a plus

• Ability to maintain confidentiality and handle sensitive situations professionally
