Full Time Physical Therapist Case Manager Anticipated Territory: Santa Rosa, CA We are offering new‑hire incentives for start dates in November 2025! What you’ll do as a Home Health Physical Therapist Case Manager: Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan. Help patients with mobility, movement, and regain their independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a quality level of care. Evaluate and report patients’ progress to MD. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (up to 4% match). As a practice, the majority of PT Case Managers are hired as PTIs. Once it is determined that a PTI can supervise two PTAs, take on a larger case load and assume a higher level of decision making and autonomy, the individual will be promoted to a PTII with a minimum salary of 135,000 plus 20K bonus (paid quarterly). #J-18808-Ljbffr
