RN - Clinical Coordinator

MDS Coordinator - Skilled Nursing Facility

This is a 13-week contract position.

Day shift, 5x8 hours, from 08:30 to 17:00.

The MDS Coordinator is responsible for completing assessments and managing care plans in a Skilled Nursing Facility. This role requires extensive knowledge of the MDS process and ongoing interaction with residents in need of short-term rehabilitation. The ideal candidate will work in a team-oriented environment, performing comprehensive evaluations and ensuring adherence to regulatory standards.

Responsibilities:

• Complete MDS assessments and develop baseline care plans.
• Conduct care plan meetings and facilitate care plan development.
• Manage the submission and validation of assessments.
• Oversee certification and recertification processes.
• Educate team members on MDS changes and conduct utilization reviews.
• Review admission referrals and coordinate preauthorization for insurance.

Required Experience / Certifications / Licensure:

• Bachelor of Science in Nursing (BSN) required.
• Minimum of 3 years of current MDS experience is required.
• Current NC state nursing license required.
• BLS certification required.
• Experience in Long Term Acute Care/Skilled Nursing settings preferred.
