In this rewarding role, you'll make a meaningful difference in the lives of individuals by providing hands-on support in a residential setting. Responsibilities: Provide direct care and daily living support to residents in a residential facility setting Assist individuals with personal hygiene, meal preparation, and household tasks Implement individualized support plans and document progress accurately Administer medications and monitor health needs in accordance with care plans Facilitate community integration activities and encourage resident independence Respond to behavioral and crisis situations using approved intervention techniques Collaborate with case managers, families, and healthcare professionals to ensure quality care Requirements: High school diploma or GED required; degree in social services, psychology, or related field preferred Valid Ohio driver's license with a clean driving record CPR/First Aid certification or (willingness to obtain upon hire) Strong communication, documentation, and interpersonal skills Ability to pass a background check and drug screeningCompassionate, patient, and committed to person-centered care About Us: Inclusion Health Services is a trusted provider of residential and community-based support services in the Columbus, OH area, dedicated to improving the quality of life for individuals with disabilities and complex needs.
