We welcome all healthcare professionals to join our thriving community and contribute to our pursuit for clinical excellence.**Department:****Job Description Summary:**Please note this position is onsite## Position ResponsibilitiesVirtual Responsibilities* Conduct virtual admissions, document patient history, allergies and baseline needs in EMR.* Uses independent critical judgement in all aspects of patient care delivery.* Provides support to bedside care team through direct observation via video monitoring and documents as appropriate.* Rounds virtually for assigned patients and documents patient response per established guidelines in the EMR.* Performs assessments on assigned patients in collaboration with bedside nurses as evidenced by complete documentation* Successfully completes ACLS training and maintains current status within 90 days of hire.* Establishes an effective communication plan with bedside nurses/PCTs to address changes in patient condition.* Follows organization's chain of command in appropriate circumstances* Demonstrates understanding of patient multi-system complications, evidenced by identification and reporting of abnormal findings to appropriate physician in a timely manner and implementation of nursing interventions.* Collaborates with all members of the health care team including physicians, when planning patient care and documents accordingly.Clinical Support* Performs virtual admissions, including history taking, medication reconciliation, preferred pharmacy.* Obtains and documents a complete nursing admission assessment, including physiological and psychological factors to develop an interdisciplinary plan of care within eight hours of admission.* Deliver personalized patient education at admission and discharge using telehealth platform.* Conduct virtual rounding to assess patient needs and escalate concerns to bedside nurse.* Assesses patient education needs on admission and daily.* Assesses psychosocial needs specific to patient's age, growth, and development.Technology Utilization* Use EPIC EMR to document virtual interactions.* Leverage Virtual Platform technology and real time communication tools.* Communication and collaboration* Act as a liaison between patients and care team.* Communicate with bedside RN, providers, and case managers as indicated.* Promote patient safety and education through virtual support.* Support bedside staff by co-signing admission/discharge documentation as required* Participate in medication reconciliation, safety checks, and patient teaching through Virtual Platform* Provides individualized instructions each shift to patient/significant other regarding the plan of care.* Demonstrates safe and correct use of unit specific equipment. (Virtual platform)* Perform discharge education and instructions via secure video, ensuring patient/family comprehension and addressing questions.Evaluation* Documents/reports patient responses or outcomes resulting from: nursing interventions, treatments/procedures, interpersonal interactions, and patient/family education.Professional and Staff Development* Obtains 100% of mandatory hospital and unit specific in-services annually, prior to evaluation date.* Plans, presents, and documents unit inservice as required by level.* Documents competency in clinical skills annually.* Attends staff meetings as required.* Uses established lines of communication for resolution of problems and interpersonal conflict.## Qualifications* ### License/Registration/Certification + Current FL Nursing License, Current Basic Life Support and Current Advanced Cardiac Life Support required* ### Education + Graduate from an Accredited School of Nursing* ### Experience + Proficient with Windows-based computer skills and operations.
