Hospice RN Case Manager: Compassionate Care Leader
Optum
Northern Arizona Hospice, part of the UnitedHealth Group family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients and collaborate with a multidisciplinary team. You will evaluate performance, implement treatment strategies, and support families facing end-of-life decisions. A strong emphasis is placed on compassionate, evidence-based nursing practice. The role requires an unrestricted RN license in the state, and CPR certification. #J-18808-Ljbffr
