HOSPICE REGISTERED NURSE/CASE MANAGER

Our RN Case Managers have the lowest nurse-to-patient ratio in the valley which allows time to connect and care at a higher level. FireNote has cut our nurses clinical charting time in half and allows for more time to be spent with your patients and your family.

Our current position is for a special type of RN who will provide care to patients where they live: private homes, assisted living facilities, and skilled nursing facilities, throughout the Treasure Valley.

REQUIREMENTS:

‪Current Idaho RN license

Strong technology skills

‪Current driver’s license

Good driving record

Clean background check 

‪Reliable Transportation‪

Responsibilities include, but are not limited to: 

Comprehensive assessment, developing care plans in collaboration with the patient, their family, and case management 

Provide education, resources coordination, pain and symptom management
