About the Role:

Right at Home Des Moines is looking for a compassionate and skilled PRN Occupational Therapist to join our dedicated home care team in West Des Moines, IA. In this flexible per-diem role, you'll make a meaningful difference by helping clients regain independence and improve their quality of life - right in the comfort of their own homes. Responsibilities:

• Conduct comprehensive occupational therapy evaluations and assessments for home care clients
• Develop and implement individualized treatment plans to improve clients' functional independence
• Provide therapeutic interventions focused on ADLs, fine motor skills, and cognitive rehabilitation
• Recommend and train clients and caregivers on adaptive equipment and home modifications
• Collaborate with interdisciplinary care teams including nurses, physical therapists, and care coordinators
• Maintain accurate and timely clinical documentation in compliance with state and federal regulations
• Monitor and report client progress, adjusting care plans as needed

Requirements:

• Valid Iowa Occupational Therapist license in good standing
• Graduate of an accredited Occupational Therapy program (MOT or OTD preferred)
• NBCOT certification required
• Home health or home care experience preferred
• Strong clinical assessment and documentation skills
• Reliable transportation and valid driver's license for home visit travel
• Excellent communication skills and a patient, empathetic demeanor

Compensation: $62.00 per hour
