Clinical Social Worker/Patient Advocate
The Clinical Social Worker/Patient Advocate provides services for the patients and families of the Program in Women's Oncology to address psycho-social, environmental, and behavioral factors which have impact on the healthy functioning and/or recovery of the patients and families. The incumbent functions as a member of the health care team to provide direct clinical services including assessment and coordination of social services. The incumbent develops a network of community agencies to facilitate referrals. Performs other duties as requested. The incumbent will plan and facilitate support groups throughout the program.
Duties & Responsibilities
- Demonstrates competency, individually and as part of the multidisciplinary team, in performing clinical care and assessment of all patient populations.
- Provides clinical social work services to patients/families/significant other, which include assessment and referrals.
- Collaborates and interacts with members of health care team and other hospital staff to coordinate services.
- Maintains required patient records, including collaboration with fiscal on billing.
- Develops and maintains network of community agencies and facilities to expand resources, facilitate patient access, and provide services to patients and families.
- Plans and facilitates support groups across the program.
- Participates in development and implementation of programs, policies and procedures.
- Preserves and protects volunteers and patients' rights to confidentiality.
- Performs other job-related duties as required or requested.
Requirements
- Master's degree in social work and a license as a Clinical Social Worker (CSW) or if hired prior to 5/2/2011 a Licensed Mental Health Worker (LMHW) required and a license as a Licensed Independent Clinical Social Worker (LICSW) preferred.
- Must have first-hand knowledge and/or experience with Oncology patients in the cancer treatment setting.
- Excellent interpersonal and communication skills required.
- Supervisory and administrative experience helpful.
- Group facilitation experience is preferred
