Housing First Case Manager — Permanent Supportive Housing

Step-Up

Step Up in Atlanta is seeking a Case Manager to provide engagement and housing stability support as part of a Rapid Housing Initiative Permanent Supportive Housing team. You will work with adults experiencing chronic homelessness to maintain permanent housing, using Housing First and Harm Reduction approaches, with home visits and telehealth as needed. Responsibilities include intake and assessment, service plan development, linking to clinical and community resources, coordinating care, and #J-18808-Ljbffr Step-Up
