Licensed Practical Nurse 6p-6a (Mon-Thurs)

As a key member of our nursing team, you will play a vital role in delivering around-the-clock skilled nursing care to our patients. Responsibilities:

• Provide high-quality patient care in a fast-paced post-acute rehabilitation setting

• Collaborate with multidisciplinary healthcare teams to develop and implement individualized care plans

• Administer medications, treatments, and other medical interventions as prescribed by physicians

• Monitor patient vital signs, symptoms, and conditions, and report changes to the healthcare team

• Assist with patient transfers, ambulations, and other mobility exercises

• Maintain accurate and up-to-date patient records and documentation

• Support the care team in maintaining a clean, safe, and organized clinical environment

Requirements:

• Current LPN/LVN licensure in the state of Louisiana

• High school diploma or equivalent required; Associates or Bachelor's degree in Nursing or related field preferred

• Prior experience in post-acute rehabilitation or long-term care setting preferred

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly

• Strong attention to detail and ability to maintain accurate records
