Home Health Case Manager – Physical Therapist Join our team at CarePartners, a leading healthcare organization serving western North Carolina. In this role, you will deliver high‑quality, patient‑centered care by managing the therapeutic process for a group of patients in coordination with physicians and the patient care team. Responsibilities Provide evaluations and treatment interventions within the scope of practice established by law and professional ethics. Assess and evaluate therapeutic, rehabilitative, and functional status and develop the total plan of care. Deliver purposeful and skilled therapeutic care at patients’ homes, using procedures and techniques aligned with the physician’s plan of care. Instruct patients, families, and caregivers in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions. Prioritize caseload, determine daily staffing needs, and coordinate assignments and handoffs. Qualifications Licensed Physical Therapist (license obtained within 30 days of employment).
