Guardian Support Staff

Position Summary
The Guardian Support Staff provides compassionate, person-centered support to individuals under guardianship by assisting with daily needs, coordinating services, advocating for their well-being, and helping ensure their health, safety, and quality of life. This role works closely with guardians, healthcare providers, community agencies, and family members to support each individual's goals while promoting dignity, independence, and self-determination.

Key Responsibilities

  • Assist guardians in monitoring the health, safety, and well-being of individuals under guardianship.
  • Maintain regular contact with clients through home, hospital, residential, or community visits.
  • Coordinate medical, behavioral health, housing, and social service appointments and follow-up care.
  • Advocate for clients' rights, preferences, and best interests in collaboration with service providers and care teams.
  • Monitor service delivery to ensure clients receive appropriate and quality care.
  • Maintain accurate, timely, and confidential documentation of client contacts, assessments, and case activities.
  • Communicate effectively with healthcare professionals, community agencies, family members, and legal representatives.
  • Assist with crisis intervention and respond appropriately to urgent client needs.
  • Support guardians with case management activities, documentation, and compliance with agency policies and applicable regulations.
  • Participate in team meetings, training, and ongoing professional development.

Qualifications

  • High school diploma required; associate's or bachelor's degree in social work, human services, psychology, or a related field preferred.
  • Experience working with older adults, individuals with disabilities, or vulnerable populations preferred.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to maintain confidentiality and exercise sound judgment.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Valid driver's license and reliable transportation may be required.

Knowledge, Skills, and Abilities

  • Understanding of person-centered care and client advocacy.
  • Ability to build trusting relationships with diverse populations.
  • Strong time management and multitasking skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Knowledge of community resources and social service systems is an asset.
