Registered Nurse Care Manager

We're seeking an experienced RN Care Manager to join a collaborative acute care team focused on care coordination, utilization review, and safe discharge planning. Qualifications Required MUST have hospital discharge planning and/or acute care management experience 2+ years of experience in an acute care hospital Preferred PRI Certification (or ability to obtain within 3 months of hire) 3+ years of Emergency Department, Critical Care, or Medical‑Surgical nursing experience Knowledge of healthcare financial, regulatory, and payer requirements Knowledge of state, local, and federal healthcare programs Experience using Milliman and/or InterQual criteria What You'll Do Complete comprehensive patient assessments and identify discharge planning needs from admission through discharge. Coordinate multidisciplinary care and collaborate with physicians, nurses, social workers, and community providers. Develop and implement safe discharge plans to reduce readmissions and improve patient outcomes. Perform utilization review and ensure compliance with payer and regulatory requirements.
