Job Summary:

We are seeking an exceptional Social Service Director to join our team at Onyx Care of Natchitoches. As a Social Service Director, you will play a vital role in ensuring the highest quality of care for our patients. You will lead our social services team in providing comprehensive and compassionate support to our patients, families, and staff. Responsibilities:
  • Lead and manage the social services team, providing guidance and support to ensure excellent patient care
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Conduct assessments and evaluations to identify patient needs and develop strategies for support
  • Coordinate with other departments to ensure seamless transitions and optimal patient care
  • Monitor and report on patient outcomes and quality of care indicators
  • Foster a positive and supportive work environment, promoting open communication and teamwork
Requirements:
  • Minimum of [ Minimum Years of Experience ] years of experience in social work or a related field
  • [ Level of Education ] degree in social work, counseling, or a related field
  • Strong leadership and management skills, with experience in supervising a social services team
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and staff
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
  • Strong analytical and problem-solving skills, with the ability to interpret data and develop effective strategies
