Work Environment

This physical therapist role is primarily onsite, working directly within patients' homes. You'll be part of a collaborative team that includes skilled nursing professionals, occupational therapists, and speech-language pathologists. You will enjoy a flexible schedule that typically includes Monday to Friday hours with occasional rotating weekend shifts to ensure comprehensive patient coverage.

Key Responsibilities

• Conduct thorough evaluations for patients experiencing functional limitations or disabilities due to injury or illness, determining individualized treatment plans based on physician's orders.
• Set measurable intervention goals and implement therapeutic strategies tailored to each patient's current physical state and personal health objectives.
• Document all aspects of patient care, including evaluations, treatment protocols, and progress notes, ensuring compliance with professional standards and regulatory requirements.
• Supervise and mentor physical therapy assistants and support staff, fostering an environment of learning and providing guidance on best practices in patient care.
• Collaborate with a multidisciplinary team, including physicians and nursing staff, to refine intervention strategies and share patient insights that enhance the overall treatment programs.
• Continuously assess and adjust treatment plans based on patient progress and changing health status, ensuring optimal outcomes and patient satisfaction.

Patient Population / Client Focus

The primary patient population includes individuals in their homes who are recovering from surgery, managing chronic conditions, or dealing with disabilities that affect their mobility and daily activities. Many patients may require specially designed treatment to address the challenges posed by their environments, necessitating creative and adaptive therapeutic solutions. You will work closely with diverse patients of all ages, fostering relationships that support their healing journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited university.
• Active Physical Therapist license in the state of Virginia.
• Exceptional communication skills, both written and verbal, are required to convey treatment plans clearly to patients and collaborate effectively with other healthcare professionals.
• Proficiency in utilizing electronic health records and documentation software to maintain accurate patient records.

Preferred Experience

• A minimum of two years of experience in a Physical Therapy setting, preferably in home health.

• Strong problem-solving skills to effectively manage patients' varying rehabilitation needs.

Compensation and Benefits
