Physical Therapist Home Health - PRN

Overview Bon Secours Home Care and Hospice by Compassus is seeking a Home Health Physical Therapist to work one-on-one with patients in their homes, helping them regain strength, mobility, and independence. Responsibilities Provide physical therapy services in patients' homes, including assessment, treatment planning, and implementation of therapeutic interventions. Work closely with patients to facilitate participation in desired daily activities (e.g., attending church, gardening). Collaborate with interdisciplinary team to ensure quality of care.
