Physical Therapist (Home Health)

• Location/Type: Greater St. Louis Region • Missouri + Metro East Illinois • Field-Based
• Coverage Areas: St. Louis County • St. Louis City • St. Charles County • Jefferson County • Franklin County • Metro East Illinois
• Pay: $45-$55/hr
• Schedule: Full-time, Part-time, PRN, or 4-day week options

Manage a local caseload with steady hourly pay, paid charting time, and fewer daily visits.

What you'll do

• Deliver 1:1 PT care in patient homes
• Build clear, goal-based treatment plans
• Improve mobility, strength, and independence
• Coordinate with RN, OT, and care teams
• Complete notes during paid work hours
• Manage 5-6 visits daily
• Adjust care plans based on patient progress

Must-haves

• Active Missouri Physical Therapist license
• DPT or equivalent Physical Therapy degree
• Valid driver's license and reliable vehicle
• Ability to work independently in the field

Nice to have

• Home health experience
• OASIS documentation exposure
• Medicare patient experience
