Registered Nurse RN Case Manager Home Health Honesdale, PA (Wayne County, Hawley, Carbondale Area) Interim HealthCare of Scranton $38-$40/hour + $5,000 Sign-On Bonus + Weekly Pay Immediate opening for a Registered Nurse RN Case Manager Home Health in Honesdale, PA and surrounding areas. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community. Pay and Benefits

38-$40 per hour
5,000 sign-on bonus

Weekly pay with direct deposit Mileage reimbursement PTO and paid holidays Health, Dental, AFLAC insurance, Retirement Plan (company match up to 3%) Schedule Monday through Friday 8:30 AM to 5:00 PM On-call, weekend, and holiday rotation Responsibilities Manage a home health caseload in Honesdale and surrounding areas Perform RN assessments and reassessments Develop and update care plans Coordinate care with physicians and healthcare team Complete home health documentation and ensure compliance Educate patients and families Requirements Active Pennsylvania Registered Nurse RN license Minimum 1 year home health RN experience Case Manager or Case Management experience preferred CPR certification Valid driver’s license and reliable transportation ABOUT INTERIM HEALTHCARE OF SCRANTON Interim HealthCare of Scranton is a locally owned home health agency serving Honesdale, Wayne County, and surrounding NEPA communities. We provide consistent patient assignments, manageable caseloads, and strong clinical support for our Registered Nurse RN team.
