Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St. John, IN and throughout Lake County . We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. $5,000 retention bonus now being offered . Retention Bonus is paid out in increments when milestone hours are met. Schedule Available: Monday - Friday. This position requires (1) on call night shift weekly.
