Providence Life Services
Providence Hospice, a division of Providence Life Services, seeks an RN Hospice Case Manager in St. John, IN. You will provide skilled nursing and case management to patients and families at end of life, traveling to patients' homes or facilities while partnering with an experienced hospice team. The role requires an Indiana RN license, at least 1 year of hospice or home health experience, and a valid driver’s license.
