Registered Nurse (RN) – Case Manager  - Travel Position (with $7,500 Completion Bonus)

Location: West Hollywood, CA
Job Type: Travel Contract
Duration: 13 weeks
Shift: 5x8 Days
Pay: $2,400.80/week (plus $2,500 completion bonus per contract, up to $7,500 after three contracts)  

About the Role

We are seeking an experienced RN Case Manager to join our care coordination team. The RN Case Manager will oversee patient care plans, ensuring continuity of care across multiple settings. This role focuses on advocacy, resource management, and achieving optimal health outcomes for patients.

Key Responsibilities

• Assess patient needs and develop individualized care plans.
• Coordinate care transitions between inpatient, outpatient, and home settings.
• Collaborate with physicians, social workers, and interdisciplinary teams to ensure comprehensive care.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on treatment options, discharge planning, and community resources.
• Ensure compliance with regulatory requirements and documentation standards.
• Advocate for patients to optimize quality of care and cost-effectiveness.

Qualifications

• Education: Associate or Bachelor’s degree in Nursing (BSN preferred).
• Licensure: Current RN license in the state of practice.
• Certifications: BLS required; CCM (Certified Case Manager) preferred.
• Experience: Minimum 2 years of clinical nursing experience; case management experience strongly preferred.
• Strong communication, organizational, and problem-solving skills.
