Position Details

• Location: Fort Collins, CO
• Schedule: MondayFriday, 8:00 AM 5:00 PM (No nights!)
• Status: Full-time (40 hours/week)

Salary: $85,508 $93,620 annually

Plus Our Built-In Pay Advantage Program:

• Scheduled 72 hours in a 2-week pay period, get paid for 80
• Mileage reimbursement at $0.625/mile (among the highest in the industry)

Your Role: Independence with Support

As an RN Case Manager, you will:

• Lead and coordinate care throughout the hospice journey
• Build meaningful connections with patients and families
• Collaborate with physicians, NPs, CNAs, social workers, and clinical leaders
• Provide holistic careaddressing physical, emotional, and spiritual needs
• Work independently in the field while backed by a highly supportive team

What We're Looking For:

• Graduate of an accredited school of nursing (BSN preferred)
• 12 years of experience in hospice, palliative care, home health, med-surg, oncology, public health, or acute care
• Current Colorado RN license and CPR certification
• Valid Colorado driver's license, auto insurance, and ability to travel daily
• Ability to lift up to 35 lbs
