Home-Based Social Worker - Assessments & Counseling

San Ysidro Health

San Ysidro Health in San Diego is seeking an In-Home Assessment Social Worker to perform in-home assessments, provide case management, counseling, and referral services. You will support Bachelor-level social workers with mandated reporting and care planning within our Day Center setting. The role requires a Master’s in Social Work, a CA driver’s license, and strong organizational and communication skills to serve diverse communities across our network. #J-18808-Ljbffr San Ysidro Health
