Transitions Care
Transitions Care is seeking a compassionate LMSW Social Worker to join our hospice team. In this role, you’ll provide psychosocial assessments, counseling, and resource coordination to patients with serious or terminal illness and their families. You’ll collaborate closely with an interdisciplinary team to support symptom management, advance care planning, and end-of-life decision making. Responsibilities include care planning, crisis intervention, grief support, and advocacy to promote dignity, comfort, and quality of life across home and facility-based settings.
