Key Responsibilities\n

• Conduct comprehensive assessments to identify the social and emotional needs of students and their families, facilitating timely interventions and support.

• Develop individualized care plans and discharge strategies in collaboration with interdisciplinary team members, including educators, psychologists, and healthcare providers.

• Actively communicate patient needs and progress to the treatment team to advance care and discharge objectives.

• Maintain organized and timely documentation in the patient electronic medical record system, ensuring compliance with healthcare regulations and confidentiality standards.

• Engage in ongoing skills training, competency development, and professional growth to remain updated on best practices in clinical social work.

Patient Population / Client Focus\n

Qualifications\n

Compensation and Benefits\n
