Essential Job Responsibilities Business Development Analyzes market trends and referral patterns of physicians and prioritizes time to increase business and drive increased census. Works with the marketing department to identify patient and referral source needs and develop education opportunities through new collateral materials and presentations. Develops and grows relationships with current and potential referral sources including case managers, therapists, provider and resident groups, and insurance case managers. Develops and executes strategic outreach plans in collaboration with the system continuum access team. Follows catastrophic cases throughout the assigned territory by rounding on appropriate call points to incrementally develop referrals. Coordinates meetings and calls on key referring physicians to drive census growth as a member of the Access team. Targets and builds relationships with catastrophic case managers. Provides reporting trends and activity to the Access Leadership Team, emphasizing barriers to care and solutions to drive growth. Admissions For referrals appropriate for admission to the system, collects patient information, completes required documentation, provides patient and family education, collaborates with hospital staff, determines admission plan and schedule, ensures compliance with CMS prerequisites, maintains effective working relationships with referral sources, facilitates communication, promotes teamwork, monitors satisfaction, and supports problem‑solving with referring physicians, patients, families, and referral sources. Other Job Responsibilities Maintains current knowledge about each referral source and associated patient populations, manages credentialing certification, and completes additional duties as assigned. Customer Service Responsibilities Demonstrates excellent customer service and behavioral standards, encourages and coaches team members, promotes teamwork and direct communication, handles confidential information discreetly and sensitively. Quality Improvement Responsibilities Contributes by identifying problems and seeking solutions, promotes patient/family satisfaction, and participates in departmental efforts to monitor and report customer service. Qualifications Active degree in nursing, physical therapy, occupational therapy or speech therapy; applicable state licensure; minimum of two years clinical experience in inpatient rehab, acute care or post‑acute setting; proficiency in clinical documentation in electronic medical record platforms; strong interpersonal, organizational and communication skills; valid driver’s license with no restrictions. Preferred Qualifications Previous experience in liaison, care management, clinical outreach or other access roles; experience with inpatient rehab, LTACH or SNF/SAR referral processes; knowledge of CMS guidelines and insurance authorization practices; demonstrated ability to analyze territory metrics and referral data; strong presentation skills and comfort speaking in front of key referral sources; ability to respond to variable workload demands. Physical Requirements Needs to remain stationary most of the time, traverse or move around work location occasionally, use keyboard majority, operate or use department specific equipment none, lift up to 10 or 25 pounds occasionally, no major lifting requirements.
