Hospice Aide

Job Description Job Description About the Role: Solara Hospice LLC is looking for a compassionate and dedicated Hospice CNA to join our caring team in Waltham, MA. Responsibilities: Provide personal care services to hospice patients including bathing, grooming, dressing, and oral hygiene
Assist patients with mobility, transfers, and positioning to ensure comfort and safety
Monitor and report changes in patient condition to the supervising nurse or care team
Deliver compassionate end-of-life care in alignment with each patients individualized care plan
Support patients and families with emotional comfort and a reassuring presence
Document patient care activities accurately and in a timely manner
Collaborate with interdisciplinary hospice team including nurses, social workers, and chaplains
Requirements: Current and valid CNA certification in the state of Massachusetts
Prior hospice, palliative care, or home health experience preferred
Strong understanding of end-of-life care principles and patient dignity
Excellent communication and interpersonal skills with patients and families
Reliable transportation and ability to travel to patient homes/long term care facilities in the Waltham, MA area
Ability to work independently while remaining a collaborative team member
Compassionate, patient, and emotionally resilient demeanor
