Home Health Aide / Certified Nursing Assistant (CNA)The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) is a paraprofessional member of the home care team who provides personal care services to patients under the supervision of a Registered Nurse or therapist.In this role, you will support patients with daily activities, monitor their condition, and communicate observations to the clinical team to ensure safe, high-quality care.Responsibilities:Provide personal care assistance such as bathing, grooming, dressing, and mobility supportObserve and report changes in patients' condition to the supervising nurse or therapistDocument care provided and patient observations accuratelyFollow the care plan established by the Registered NurseMaintain a safe and supportive environment for patients in their homesPromote quality, continuity, and safety of patient careQualifications:Certified Nursing Assistant (CNA) in Massachusetts preferred; Home Health Aide certification acceptedMust be 18 years of age or olderAbility to read and follow written care plans and document services providedAbility to work independently with minimal supervisionCompassion and empathy for individuals who are ill, injured, or elderlyCurrent CPR certification requiredStrong interpersonal skills, patience, and professionalismValid driver's license and reliable insured vehicleAbility to assist patients with repositioning, transferring, and other physical tasks.Pathways Healthcare is committed to providing exceptional care to our patients and fostering a positive work environment for our team members.
