Deliver evidence‑based therapy services in a skilled nursing and rehabilitation setting. Monitor resident progress and adjust treatment plans to maximize outcomes. Collaborate closely with nursing, physicians, and the interdisciplinary care team. Educate residents and families on treatment goals and therapeutic techniques. Maintain accurate and timely clinical documentation. Ensure compliance with all state, federal, and professional practice standards. Help residents improve mobility, function, independence, and overall quality of life. What We’re Looking For Current Colorado Physical Therapist (PT) or Occupational Therapist (OT) license, or eligibility for Colorado licensure. Graduate of an accredited Physical Therapy or Occupational Therapy program. Skilled Nursing Facility (SNF) or Long‑Term Care (LTC) experience is preferred but not required. Strong clinical reasoning, communication, and documentation skills. A compassionate clinician who enjoys working collaboratively and making a lasting impact.
