Overview Clallam County, Washington $40K Student Loan Repayment OR $30K Sign-On Bonus About the Role Optum Home & Community Care is transforming how care is delivered. As part of our HouseCalls team, you’ll provide preventive, in-home health assessments—helping patients stay well while connecting them to the care and resources they need. This is a non-prescribing, field-based role focused on improving health outcomes through education, gap closure, and collaboration with primary care providers. What You’ll Do Conduct in-home health assessments (history, meds, vitals, physical exam) Perform screenings and point-of-care testing (as appropriate) Identify and document conditions for care planning Communicate findings to PCPs to close care gaps Educate patients on conditions, medications, and next steps Address social needs and connect patients to resources Recognize urgent issues and escalate as needed Required Qualifications NP: ANCC/AANP certification (Family, Adult, Geri, or AGPCNP) PA: NCCPA certification Active, unrestricted state license (or ability to obtain) Valid driver’s license + reliable transportation Ability to perform in-home visits (including mobility and lifting up to 30 lbs) Preferred 1+ year clinical experience (primary care, geriatrics, or home health) Comfort with EMR/technology Experience working with Medicare/geriatric populations Why Join Optum Strong support and autonomy in the field Clear expectations and growth opportunities Mission-driven work improving patient access and outcomes At UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
