Work Environment

As a Physical Therapist at Strive Physical Therapy, you will work in a supportive and dynamic onsite environment within our well-equipped Rehabilitation Center in Fremont. The facility is designed to foster collaboration among healthcare teams, ensuring patients receive comprehensive rehabilitation services. Our operating hours are from Monday to Friday, 9 am to 5 pm, allowing for a structured yet engaging work schedule. You will work closely with our multidisciplinary team to facilitate optimal patient care.

Key Responsibilities

• Conduct thorough assessments to identify patient goals, needs, and functional limitations.
• Manage a patient load of one individual per hour, ensuring focused and effective treatment.
• Deliver high-quality orthopedic rehabilitation services, leveraging your expertise and previous outpatient care experience.
• Formulate individualized care plans using evidence-based practices and a mix of treatment techniques.
• Implement and adapt innovative treatment strategies to enhance patients' mobility and reduce dependence on pain medications.
• Develop and customize fitness and wellness programs in alignment with patients' specific recovery goals and lifestyles.
• Evaluate patient progress actively, documenting all interventions and patient outcomes clearly and accurately.
• Maintain open lines of communication with patients, ensuring they understand their treatment plans and objectives.
• Encourage and motivate patients throughout their rehabilitation journey, enhancing their engagement in therapy.
• Work collaboratively with other healthcare professionals, sharing insights and strategies to adjust treatment as necessary.

Patient Population / Client Focus

We cater primarily to adult patients needing orthopedic rehabilitation, including those recovering from surgery, injury, or chronic conditions affecting mobility. Your day-to-day interactions will lead you to work closely with individuals who seek to regain their prior levels of function and improve their quality of life through effective rehabilitation techniques.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current license to practice as a Physical Therapist in California.
• No disciplinary actions or board actions against your professional license.
• Proven manual therapy skills and a strong foundational knowledge of therapeutic practices.
• Experience in creating and implementing effective exercise routines for diverse patient populations.
• Proficient in using Electronic Medical Record (EMR) systems such as WebPT.

Preferred Experience

• A minimum of 3 years of experience working as a Physical Therapist in an orthopedic outpatient setting.
• Familiarity with advanced treatment techniques and modalities, including manual therapy and therapeutic exercise.
• Experience in collaborating within multidisciplinary teams to enhance patient care.
