Care Manager / Social Worker (BSW)

Location: Salina, KS

Employment Type: Full-Time

• Care Manager: Starting at $30.11/hour (based on education and experience)

• Social Worker (BSW): Starting at $24.77/hour (based on education and experience)

• Relocation Assistance: Up to $5,000 (subject to eligibility)

We are seeking a compassionate and detail-oriented Care Manager or Bachelor of Social Work (BSW) to join our care management team. In this role, you will coordinate patient care across the continuum, ensuring timely, appropriate, and cost-effective services while promoting positive health outcomes. You will collaborate with patients, families, physicians, and interdisciplinary healthcare teams to assess needs, develop individualized care plans, and facilitate safe transitions of care.

• Coordinate and manage patient care plans throughout the continuum of care.

• Assess patients' medical, psychosocial, and discharge planning needs.

• Develop, implement, monitor, and evaluate individualized care plans.

• Ensure appropriate level of care based on medical necessity and clinical guidelines.

• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate services.

• Facilitate discharge planning and connect patients with appropriate community resources.

• Educate patients and families regarding available services, treatment plans, and follow-up care.

• Advocate for patients while promoting quality, safety, and efficient utilization of healthcare resources.

• Maintain accurate and timely documentation within the electronic medical record.

• Participate in interdisciplinary team meetings and case reviews.

• Foster positive relationships with patients, families, and healthcare team members through respectful and compassionate communication.

• Bachelor's degree in Social Work, or another healthcare-related field (or as required by state regulations).

• Previous experience in care management, case management, utilization review, discharge planning, or a related healthcare role.

• Strong understanding of healthcare delivery systems, medical necessity, and care coordination.

• Excellent assessment, critical thinking, communication, and organizational skills.

• Ability to effectively collaborate with interdisciplinary healthcare teams.

• Bachelor's Degree in Social Work (BSW) from an accredited program.

• Current licensure LBSW LISW LCSW or eligibility for licensure as required by state regulations.

• Experience in hospital, healthcare, case management, or social services preferred.

• Strong knowledge of psychosocial assessments, discharge planning, and community resources.

• Excellent communication, advocacy, and care coordination skills.

• Acute care or hospital experience.

• Experience with utilization management and discharge planning.

• Familiarity with electronic medical record (EMR) systems.

• Knowledge of Medicare, Medicaid, and insurance authorization processes.

• BLS certification, if applicable.
