About the Position: - Work closely with a dynamic interdisciplinary rehabilitation team. - Treat a diverse population with orthopedic, neurological, cardiopulmonary, and medically complex conditions. - Help residents regain independence and enhance their quality of life through meaningful occupational therapy. Responsibilities: - Provide skilled occupational therapy treatments under the supervision of a licensed Occupational Therapist. - Implement personalized treatment plans to improve activities of daily living (ADLs), functional mobility, upper extremity function, cognition, and safety awareness. - Document treatments accurately using electronic medical records. - Collaborate with the interdisciplinary team regarding patient progress and discharge planning. - Educate patients, caregivers, and facility staff about adaptive equipment, home safety, and functional strategies. - Maintain compliance with Medicare, state, and facility documentation standards. - Participate in care conferences and team meetings as appropriate. Qualifications: - Graduate of an accredited Occupational Therapy Assistant program. - Current California COTA license (or eligibility). - Current CPR/BLS certification. - Skilled Nursing Facility experience preferred; motivated new graduates welcomed to apply. - Excellent communication and teamwork skills. - Positive attitude with a passion for patient-centered care.
