Schedule: Monday to Friday, 8:00 AM - 5:00 PM

About the Role
We are seeking a compassionate Primary RN - Case Manager for a 13-week contract opportunity with HopeWest Hospice in Grand Junction, Colorado. This role involves managing an assigned hospice caseload, leading an interdisciplinary care team, and providing exceptional end-of-life care in patients' homes, nursing homes, assisted living facilities, and other community settings.
Responsibilities
• Manage an assigned caseload of hospice patients and coordinate individualized care plans
• Provide skilled nursing care, symptom management, and patient assessments
• Educate patients and caregivers on disease progression, symptom management, and end-of-life care
• Collaborate with the interdisciplinary team to ensure quality, patient-centered hospice care
• Document patient care accurately and participate in occasional weekend backup and on-call coverage
Requirements
• Active Registered Nurse (RN) License required
• Minimum 2 years of Hospice RN experience required
• Strong clinical assessment, critical thinking, and case management skills
• Experience providing end-of-life and symptom management care
• Excellent communication, patient education, and interdisciplinary collaboration skills
