Physical Therapist- Full Time Opening

As the lead Physical Therapist for two skilled nursing facilities in St Petersburg, FL. Our SNF’s offer IN-HOUSE Comprehensive Rehabilitation Programs 6 days a week. Each facility has a rehab gym, and the rehabilitation team led by an experienced Rehab Director and a team of experienced clinicians. Each rehab department utilizes Net Health software for billing and documentation to assist therapists with daily efficiency where quality care remains the primary focus of the Physical Therapist.

Our Regional Director of Rehab helps support the rehab department with operations, clinical services, mentorship, development, compliance, and recruitment.

PHYSICAL THERAPIST QUALIFICATIONS:

• Graduation from an accredited Physical Therapist Program
• Professional State Licensure/Eligibility
• LTC/SNF Experience Preferred
• Professional Appearance and Attitude
• Customer Service Oriented
• Knowledgeable of PDPM and LTC program development

PHYSICAL THERAPIST JOB DESCRIPTION:

• Responsible for Resident Physical Therapy Evaluations and Re-certifications
• Completion of required Physical Therapy Documentation (Progress Notes, Discharge Notes, Treatment Encounter Notes, Section GG, Screenings, Referrals to Therapy)
• Communication patient progress or problems to supervisor and other team members; assist with patient scheduling and daily billing of resident records.
• Physical Therapy Clinical Programming and compliance with department policy and procedures
• Supervision of Physical Therapy Assistants
• Responsible for individual productivity and efficiency standards.
