Physical Therapist – Skilled Nursing Facility, Nashua, New Hampshire, United States. Start date 06/29/2026 for a 14-week travel assignment with guaranteed 40 hours per week. The role follows a 5x8 Days shift schedule (08:00–16:30) in a long-term care, skilled nursing environment.

Responsibilities
- Conduct comprehensive PT evaluations and develop individualized plans of care for residents in a skilled nursing, long-term care setting.
- Implement PT interventions including therapeutic exercises, gait and balance training, manual therapy when appropriate, and transfer training.
- Monitor patient progress, perform re-evaluations, and adjust treatment plans to optimize outcomes; document in the electronic medical record.
- Collaborate with an interdisciplinary team (nursing, rehabilitation therapy, social work) to coordinate care and discharge planning.
- Ensure patient safety, infection control, HIPAA compliance, and patient/family education regarding home exercise programs and progression.

Requirements
- Active, unrestricted Physical Therapist license in New Hampshire
- Doctor of Physical Therapy (DPT) degree or equivalent
- Relevant experience in a skilled nursing or rehab setting preferred
- Ability to work in a dynamic, multi-disciplinary environment
- Eligibility to work in the United States and comply with travel assignment requirements
