Responsibilities The Speech Language Pathologist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient’s private residence or in a patient’s room in an assisted living facility, or in an independent living facility. Evaluate and treat in the most functional environment which is the patient’s home. Organize and provide speech-language services for the agency to facilitate rehabilitation. Direct patient participation in selected tasks to restore, reinforce, and enhance performance. Facilitate the learning of skills and functions essential for communication and swallowing. Direct activities to promote and maintain health, within the scope of practice and consistent with the Code of Ethics of the profession. Provide a flexible schedule and work environment in various settings. Supervision of CFYs, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards (as necessary or applicable). Adhere to department and patient treatment schedules, complete timely clinical and billing documentation, and participate in work-related meetings. Attend and contribute to patient care, staffing conferences, and related meetings. Make referrals to other disciplines or agencies to address comprehensive patient needs. Collaborate with rehabilitation team members and physicians to select appropriate therapeutic regimens; adapt plans of care as needed. Promote a safe environment and enforce the elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean and maintain equipment and report repairs as needed. Put patient service first, ensuring residents and families receive high-quality care in a compassionate atmosphere. Screen, examine, and evaluate patients, including history and systems review; interpret findings to establish diagnoses, plan care, and set goals in collaboration with patients and caregivers. Implement speech-language pathology treatment plans; provide documentation and record keeping for services. Deliver services in areas such as cognitive-communication, speech intelligibility, oral motor skills, swallowing disorders/dysphagia, perceptual abilities, orientation, memory, pragmatics, psychosocial expression, functional communication, and case management in home and community settings. Coordinate acquisition of adaptive communication devices and instruct patients in their use. Facilitate identification of hearing deficits and refer for assessment as needed. Select appropriate activities and evaluate patient progress, attitudes, and behavior related to rehabilitative potential. Re-examine as needed to modify plans of treatment. Coordinate care in the home and community with other members of the interdisciplinary team in collaboration with the referring physician. Performs other related duties as required. Qualifications Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility. Must have a valid driver’s license or reliable transportation to and from patients’ residences. Posted Salary Range: USD $46.00 - USD $62.00 /Hr. #J-18808-Ljbffr
