Shift Notes: 0615 start 5x8 or 4x10

Weekend Requirements: 4th weekend

On Call Requirements: 1weekend Friday, Saturday and Sunday a month and roughly 4 weekday calls per month. May need to take a holiday call if coverage is needed.
