Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 06/15/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours Employment Type: Travel Physical Therapist — Outpatient — Chicago, IL Provide outpatient physical therapy services to evaluate, develop, implement, and modify individualized treatment plans that support patients' return to community activities. Delegate and direct discipline‑specific therapy services as appropriate. Complete timely, accurate documentation including evaluations, goal writing, functional assessments, letters of medical necessity, and charge entry. Participate in student education when assigned. Maintain professional licensure and adhere to applicable state practice acts and organizational policies. Demonstrate commitment to patient and employee safety and organizational core values. Minimum Qualifications Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Illinois physical therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Experience working in an outpatient setting preferred. Must be able to float between clinic locations as required by the client. Preferred Qualifications Experience with outpatient orthopedic populations and functional restoration programs. Prior experience supervising or precepting students. Familiarity with electronic medical record documentation and charge capture processes. Assignment Details Shift pattern: 5x8. Floating required.
