What You'll Do:

• Evaluate patient functional status through assessments
• Develop individualized treatment plans in collaboration with physicians
• Deliver outpatient therapy, with occasional float to acute care settings
• Educate patients and families on conditions, treatment options, and prognosis
• Supervise Occupational Therapist Assistants, aides, and students
• Recommend adaptive equipment and assist with securing necessary items
• Document patient progress in compliance with policies and regulations
• Participate in interdisciplinary team meetings and community education
• Act as case manager for assigned patients to ensure continuity of care
• Contribute to program development and quality improvement initiatives

What You Need:

• Valid Michigan Occupational Therapist license
• At least 1 year of outpatient experience preferred
• Strong assessment, communication, and collaboration skills
• Ability to work independently and as part of a team
• Commitment to patient-centered care and community engagement
