Charge Nurse LPNCarriage Court of Grove City - Grove City, OH 43123OverviewSalary Range $28.00 - $29.00 Hourly Position Type Full TimeDescriptionResponsibilities:Ensures appropriate medical follow-up providing special instructions to resident and clinical staff related to resident careCoordinates and reviews lab work, coordinating care with clinics, outside providers, and hospitalsCompletes standardized assessment and assessment tools, may conduct initial assessment on prospective residents including offsite visitsAssists to complete and manage the daily or monthly staff scheduling for the clinical departmentParticipates in and has responsibility for after hour on call weekend duties to ensure resident care is provided 24/7/365Performs assigned quarterly and annual reassessments on residents often utilizing EHR Also performs such assigned reassessments when the Resident's health condition warrants.Communicates with resident's providers, other pertinent health care providers and family as to the ongoing health status of the residents.Refers residents to appropriate medical care when needed.
