Physical Therapist Case Manager: Home Health

We require all opportunities to involve field visits to patients’ homes. What you’ll do as a Home Health Physical Therapist Case Manager Perform in‑home evaluations and develop care plans. Work with Physical Therapy Assistants to implement the care plan. Help patients improve mobility and regain independence. Coordinate care with physicians to ensure quality treatment. To be a success as a Physical Therapist Current unrestricted CA Physical Therapist license. Current BLS/CPR certification. Valid CA driver’s license. Knowledge of local community resources. Knowledge of individual development, human behavior, cultural influences on health care, and state/federal home‑care regulatory guidelines. Experience in home health care (high acuity patients) and with interdisciplinary teams. Familiarity with case‑management fundamentals. Self‑directed, empathetic, critical thinker with outstanding interpersonal and leadership skills. Compensation & Benefits $125,000 – $165,000 per year. In addition to competitive base pay, we offer generous bonuses up to $40,000 annually, paid quarterly. Wage range for Physical Therapist, Level 1: $130,000–$150,000 plus a $10,000 annual bonus, paid quarterly. Starting pay is commensurate with experience. 21st Century also provides comprehensive health benefits and a 401(k) with up to a 4% company match. PT Case Managers are hired as PTIs.
