Role Summary and Qualifications

The Physical Therapist (PT) is responsible for delivering high-quality, patient-centered care by managing the entire therapeutic process for a designated group of patients in collaboration with physicians and the patient care team. The PT conducts patient assessments and plays a key role in creating and implementing personalized care plans that focus on achieving goals, engaging patients, and enhancing their experience.

Key Responsibilities:

• Conduct evaluations and treatment interventions within the legal and ethical scope of practice.
• Assess and evaluate the therapeutic, rehabilitative, and functional status of patients while contributing to the overall care plan.
• Deliver skilled therapeutic care in patients' homes, utilizing techniques and procedures aligned with the physician's care plan.
• Educate patients, families, and caregivers on therapies, exercises, therapeutic devices, and prosthetics for use outside of therapy sessions.
• Manage caseload priorities, determine daily staffing needs, and coordinate assignments and handoffs.

Qualifications Required:

• A valid Physical Therapist license must be obtained within 30 days of starting employment.
• A Bachelor's Degree is required.
