Physical Therapist (PT) – Home Health | Eastern Cincinnati Area
$60–$110 per visit | Clermont & Brown County, OH | Flexible Schedule | Full Benefits Available
Looking for a Physical Therapist who wants more autonomy and better pay per visit than a clinic setting offers. This is a direct-hire home health role covering Clermont and Brown counties — you'll build real relationships with patients in their own homes and set a schedule that actually works for you.
What you'll be doing:
Evaluating patients and building individualized treatment plans
Providing skilled PT services in patients' homes
Adjusting care plans as patients progress
Teaching patients and caregivers safe exercises and daily living techniques
Working alongside physicians, nurses, and other care team members
Completing OASIS assessments and progress notes
Supervising PTAs as needed
What you need:
Degree from an accredited Physical Therapy program
Active PT license in Ohio
2 years of PT experience preferred (home health experience a plus, but not required)
