Job Description & Requirements Physical Therapist Therapy 12 weeks 40 hours per week 8 hours, days Travel Hospital: Centerpoint Medical Center Location: Independence, MO Address:19600 E 39th St, Independence, MO 64057 Unit Name: Physical Therapy Building: Cost Center: 76226 General Information Unit Specifics: Number of Beds : 285 acute care, 16 inpatient rehab Patient Ratios : Acute12:1, rehab 4-5:1 Unit staffing description- (unit secretaries and tech availability) : Rehab tech Type of equipment : Safe patient handling equipment, all normal rehab equipment Documentation system: Meditech Scheduling requirements: 5 8 hour days M-F, 1 weekend every 6 weeks, able to flex off days if work weekend. 0800-1630 On Call Requirements: NA Floating policy: may cover both acute and rehab Patient Experience requirements: Hiring profile Requested Skill Set: Must have: Licensed PT Advanced skills needed: Acute care/ rehab experience Procedures performed on unit: Evaluations/ Treatments Dress Code (Required uniform) Awards/Recognition: Please include anything that you would like prospective candidates to be aware of, such as, links to videos that feature the department, outstanding department/team awards..
