Clinical Pharmacist - Hospice & SNF Care Lead

UPMC is seeking a Full-Time Clinical Pharmacist to support Rx Partners At Home and Hospice/SNF pharmacy operations. The role provides clinical oversight, regulatory support, and participates in weekend, evening, overnight, and on-call rotations, contributing to quality improvement and medication management in long-term care settings.
